Advanced Practitioners

You may be offered an appointment at the minor illness clinic run by our advanced nurse practitioner. The nurse practitioner will be responsible for taking history, examination, diagnosis and treatment for minor illnesses. They are also able to prescribe and will of course refer/discuss with a doctor when appropriate.

They are highly qualified experts in medicines and can help people in a range of ways. This includes carrying out structured medication reviews for patients with ongoing health problems and improving patient safety, outcomes and value through a person-centred approach.

Nurses

At present we have a team of practice nurses, all of whom can provide a variety of services. Their numerous tasks include infant and adult immunisations, cervical smears and family planning, dressings and removal of stitches, and clinics to monitor blood pressure, asthma cardiovascular disease and diabetes. Appointments may be made by phoning your surgery after 9.30am each morning.

  • First Contact Physiotherapist

    When a patient presents with any pain coming from joints, muscles, soft tissues, nerves, etc., a First Contact Physiotherapist uses their assessment and communication skills to diagnose Musculoskeletal problems. They will then provide guidance to manage patients’ symptoms. If symptoms persist, a patient’s condition worsens, or a pattern of pain and symptoms are not MSK related, they will direct or refer patients to the most appropriate department.

  • Primary Care Mental Health Team

    The Primary Care Mental Health Team (PCMH) for Worcester City PCN (WCPCN) is a team of registered mental healthcare professionals.

    Assessment, care planning and review for anyone experiencing a mental health condition age 12-40 years. Anyone presenting with complex or multiple needs associated with a mental health condition which is being supported by more than one agency/service age 12yrs +

    Referrals can be made by any registered healthcare professional working within a practice.

Practice Management Team

 Our Practice Manager runs the practice. They are ultimately in charge of all staff and clinicians. Along with the Practice’s Partners, the Practice Manager leads the vision of the Practice- as well as ensuring contractual obligations are met. The Practice Manager is supported by an Assistant Manager.

    The Practice Manager leads the vision of the practice as well as ensuring contractual obligations are met. Charlotte is also responsible for providing operational oversight and leadership to ensure the smooth running of SJHMC Surgery. The Practice Manager assists the Partners in the management and leadership of the Practice to deliver high quality care to the patient population.

    ASSISTANT PRACTICE MANAGER

    The Assistant Practice Manager supports the Practice Manager and partners in all aspects of practice functionality. Focusing on quality, covering all aspects including service delivery and the processes required to maximise the practices performance and income and balancing that with patient satisfaction.

Receptionists and Administrators

We employ a number of staff who perform various administrative tasks and reception duties. They work in the “engine room” at the medical centre and are the people with whom you first make contact, either on the telephone or when you visit the centre. When not assisting patients, these members of staff complete reports, handle documents and ensure that patient records are up-to-date.

    The reception team leader is responsible for undertaking a wide range of reception duties and the provision of general support to the multidisciplinary team. Duties can include; greeting patients, booking appointments, the processing of information and assisting patients. While also supporting a team of receptionists.

    The senior clinical coder is the initial point of contact from secondary care, external agencies and for new patients records at the practice to the clinicians, other clinical and non-clinical staff. While also supporting a team of administrators.
